Instructions

As part of Powerhouse Engines's supplier qualification and ongoing vendor management process, all prospective and existing vendors must complete and return this questionnaire. An updated questionnaire must also be submitted whenever there is a significant change to your organization's information, including changes to company name, ownership, certifications, address, or operational status.

If your company is an approved FAA Part 145 Repair Station, please attach: Air Agency Certificate, Operations Specifications, Capabilities, current FAA Anti-Drug / AMPP (A449-1, 2) program, EASA Certificate (if applicable), CAAC Certificate (if applicable), ISO 9001 / AS9100-series, ASA-100 / FAA AC 00-56 certificate.

If your company is a distributor, supplier, or broker, please attach: Quality System Certification (ISO 9001, AS91XX, ASA-100, or other). If not certified, please provide a copy of your Quality Manual.

Return the questionnaire within 15 days of receipt to maintain your status as an approved Powerhouse Engines supplier.
